I'm with the same problem. Using the protocol T = 0, Direct. In the simulator I have no problems, but in a real card does not work. I generated 2048-bit RSA keys without problem, but when I want to generate 4096-bit key, the reader sends me an error indicating that the card does not respond, I guess that will wait too long and the reader is running out. Anyway try #pragma ATR(WI=254) to see if it works for you so.
